Your visiting agent can represent the request while the clinic's agent answers from its own records and policy. Together they prepare the exact recipient and scope; you confirm the disclosure yourself — on your own device, every time.
The conversation in the corner is the shared exchange. You can observe, answer, interrupt, and approve while both agents remain distinctly attributed.

This page lists what kind of record each visit produced. It never shows the records themselves — those are only ever sent, to someone you name, after you confirm.
Records policy
- Your records are yours. You can see what we hold and ask us to send it anywhere. We do not need a reason.
- Every release is confirmed by you, in person, each time. Nobody acting on your behalf can confirm one for you — not a family member, not an assistant, not us.
- One release, one recipient, one scope. A confirmation authorises exactly the recipient and the records named in front of you when you confirm it.
- Routine records travel together: visit summaries, labs, imaging, immunisations and prescriptions.
- Restricted records do not — at all, through an agent. Mental health, substance use and genetic records are never released through an agent-prepared disclosure. An assistant cannot include them, whatever it asks; a person who needs them must request them another way.
